I noticed that many pictures of houses with metal roofs do not have gutters. Why is that? How is all the run off from heavy rain handled - keeping it away from foundation.

ice damaged gutters. Ice slides right off and smashes the gutters all to hell

You may want to consider using Snow Guards to prevent the snow/ice from sliding off.

Some areas of the country do not use gutters as much as other areas. My metal roofed homes have always had gutters -- no problems. And I am in the snowbelt. Several inches today in fact. I do suggest 6" gutters from min .032" aluminum with internal brackets fastened into every rafter tail.
