I currently have a 5:12 roof on my house.
I am attaching a covered patio to the front and back side of the house.
The patio will be approx. 10 x 30 on both sides.
I will be reroofing the entire house with light weight concrete tiles except for the patio. Sheeting will be 5/16 radiant barrier.
I would like to install the metal panels
on the patio. Maybe 1:12 pitch.
Can this be done with the metal panels
installed on the low pitch patio and the transition from tile to metal ?

Yes but there are a number of things to consider such as how you are venting the main roof and are you going to be closing in any part of the patio.
In order for the radiant barrier to be effective it must be at least 1" away from the metal and this space should be vented. I would run a peel and stick barrier up the low slope and 4 ft up the main roof first. Then choose a standing seam panel that either stands off on clips or can be installed on strapping. Use a profiled closure strip and a large metal transition flashing wher the roof connect.
